変形性股関節症(以下変股症とする)手術例に対するケアプラン表適用が,術後ケアの向上に役立っているかどうかを検討した.変股症で人工股関節置換術(THA)を受けた症例を対象とした,ケアプラン表適用前の15症例16股関節をA群,ケアプラン表適用後の14症例14股関節のうち,下肢深部静脈血栓(疑)のない9症例をB-1群,ある5症例をB-2群とした。これら3群について,入院日数,術後ケアおよびリハビリテーションに関連した項目を比較検討した.術後初めての洗髪・チェアバス・ベッドアップ30度・松葉杖歩行までの日数は,3群間に差はなかった.しかし,チェアバス・ベッドアップ30度・ベッドアップ自由の分散を見ると,A群と比べB-1群において有意に分散が小さく,ケアの標準化がなされていたと思われた.一方,ベッドアップ自由・端坐位・車椅子までの日数は,A群各11±3日,14±1日,17±2日,B-1群各4±1日,8±2日,12±4日であり,B-1群で有意に短縮していた.また,B-1群とB-2群とを比較すると,ベッドアップ自由・端坐位・起立訓練・車椅子・歩行器歩行開始までの日数において,B-2群が有意に延長していた.以上より,変股症に対する人工股関節置換術症例では,ケアプラン表適用が,術後ケアの質の向上とリハビリテーションの促進に有効であることが示された.下肢深部静脈血栓の合併は,リハビリテーションを遅らせることが明らかとなり,今後はその発症の予防に一層努力したい. |

Since 2000, we have applied the care plan after total hip arthroplasty(THA). In this paper, the effect of the care plan implemenyation was assessed in terms of postoperative care and rehabilitation. The objects were patients who underwent THA for osteoarthrosis of the hip. Fifteen patients who underwent THA in 1997(before care plan implementation) were denoted as A group. Among 14 patents who underwent THA in 2001 (after implementation), nine without deep vein thrombosis (DVT) were denoted as B-1 group, and 5 with DVT were denoted as B-2 group. Based upon the patient records, various parameters such as length of hospital admission, postoperative care and rehabilitation process were compared among these groups. Of the postoperative days performing shampooing, chairbathing, 30° bed up and walk on cruches for the first time, there were no differences among 3 groups. However, the dispersions in these parameters were significantly smaller in B-1 group, suggesting that the standardization of patient care had been achieved in B-1 group. On the contrary, the days of free bed up, sitting on bed and sitting on wheelchair after operation were 11±3, 14±1, 17±2 in A group and 4±1, 8±2, 12±4 in B-1 group, respectively, indicating that they are significantly shortened in B-1 group. Between B-1 and B-2 group, the days of free bed up, sitting on bed, standing up practice, sitting on wheelchair, walk with a walker were significantly elongated in B-2 group. As a summary, in patients after THA, the implementation of care plan was effective in the patient cares and acceleration of rehabilitation. Furthermore, it is revealed that the complication of DVT retards the rehabilitation process significantly, so a particular effort must be made to prevent DVT occurrence. |
