平成14年8月から15年4月までに,管理栄養士が褥瘡対策チームの一員として栄養補給した17症例の成績を検討した.対象は男13例,女4例,72±8.6歳であり,全例が重篤な基礎疾患をもち,17例中13例が手術後の患者であった.20病変(17症例)の褥瘡の転帰は,治癒14病変,改善中に転院・退院2病変,不変転院・死亡3病変,悪化転院1病変であった.栄養補給の内容はアイソカルジェリー66gの補給,卵料理の追加等を行った.補給期間は1〜15週間(平均5.4週)であった.これらの補給により栄養状態が改善し早期に褥瘡が治癒した症例もあった.しかし栄養補給の前後の血清アルブミン値は,前値2.8±0.4g/dl,後値2.9±0.6g/dlであり,ヘモグロビン値も前値9.3±2.0g/dl,後値9.2±1.2g/dlであった.すなわち,両指標とも血液学的には改善は認められなかった.従って,今後は栄養補給の内容を充実し,補給エネルギーを増量する必要があるものと推察された. |

We examined treatment of 17 cases of pressure sores in hospitalized patients by the Nutrition Support Service as part of a pressure sore prevention team between August 2002 and April 2003. Of the 17 patients, the male/female ratio. was 13/4, mean age 72 years, and all suffered from serious diseases such as malignancy, heart failure or aortic aneurysm. 13 out of 17 received surgery just before the Survey. There were 20 pressure sore lesions among 17 patients. Of the outcome of these 20 lesions, 14 lesions were cured, 2 improved and the patient discharged, 3 were unchanged and the patient discharged, or the patient died, 1 deteriorated and the patient was discharged. With respect to the content of nutritional supplements, ISOCAL 66g or a boiled egg was supplemented for 1 to 15 weeks (mean 5.4 weeks) in addition to the regular meal. After these nutritional supplements, serum albumin increased and pressure sores improved abruptly in some cases. However, as a whole, the serum albumin level averaged 2.9±0.4g/dl before nutritional support and 2.9±0.6g/dl after support, and Hb averaged 9.3±2.0g/dl, and 9.2±1.2g/dl, respectively, indicating that no increase was observed statistically in these 2 parameters through nutritional supplementation. Although 16 out of 20 pressure sore lesions were cured or improved clinically, it is presumed that the caloric value of the supplement should be doubled or tripled to improve the nutritional condition of patients with pressure sores. |
