慢性疾患患者の看護において,病みの軌跡の概念が重要だといわれている.病みの軌跡を理解し,適切な管理をすれば,病気がコントロールされると考えられている.本症例は,クローン病の初期の段階において,クローン病患者に面接を行い,病みの軌跡を明らかにした.その結果,患者は,再燃の経験を繰り返していないことから病気を直視できず,社会との接触を避け,「病気を隠す」という療養行動をとり,夫に依存しながら病気を乗り越えようとしていた.今後,患者の病みの軌跡は変化するものであり,看護者は,患者との対話から,慢性疾患と共に生きるプロセスの段階を捉え,病みの軌跡に沿いながら支援するという課題が残った. |

It is said that a general idea of illness trajectory is important in the nursing of chronic illness patients: illness can be controlled if the trajectory is understood and properly managed. In this case we interviewed a patient in the initial stages of Crohn's disease, and clarified the illness trajectory. The results were that the patient had been unable to face her illness squarely from not experiencing a recurrence. She avoided contact with society, and took on a behavioural pattern of illness concealment to cope with the illness, becoming dependent on her husband. She did not prioritise nursing assistance. Because illness trajectory is transitional, from now on it remains for the nurse to support the patient as the illness progresses, initiating a dialogue with the patient in a process enabling the patient to live with the chronic illness. |
