| 内容記述 | 介護支援専門員（以下 caremanager,CM）から見た急性期病院における退院前カンファレンスの評価及び退院前カンファレンスの達成度に関連する要因を明らかにすることを目的に，東北地方の居宅介護支援事業所1,000名のCMを対象として，郵送法による質問紙調査を実施した．調査内容は，【CM の背景】・【多職種連携要因】・【退院前カンファレンスの構成要素】・『総合的に見た退院前カンファレンスの達成度』である．回収数436件のうち有効回答数426件を分析対象とした． 『総合的に見た退院前カンファレンスの達成度』は，５点満点中平均得点3.65±0.87であった．『総合的に見た退院前カンファレンスの達成度』を従属変数とした多重ロジスティック回帰分析の結果，【多職種連携要因】では『急性期病院との日常における連携のしやすさ』『情報共有回数』，【退院前カンファレンスの構成要素】では『開催タイミング』『資料の効果的活用』『チームメンバー間の信頼関係』『チームによる支援計画の具体化』『当事者の参画』が影響していることが明らかになった． 退院前カンファレンスの実質的な運営には，カンファレンスの開催時期の適切性及び資料を効果的に活用し，CMとの信頼関係の中で議論や意見交換ができること，さらにこのプロセスから患者の意思決定や強みを活かした支援計画を具体的にする運営が必要であることが示唆された． In acute hospitals, we have a pre-discharge conference. We aim to uncover the factors which are involved in the assessment and achievement of the pre-discharge conference from a view point of a care manager (CM). We conducted a questionnaire survey for 1,000 CMs working in the in-home care support offices in the Tohoku Region in Japan. We asked about【Background of CM】,【Interprofessional cooperation factors】and【Components of pre-discharge conference】for the factors which are involved in『Comprehensive achievement of pre-discharge conference』. We analyzed 426 valid responses among 436 responses. The average score of『Comprehensive achievement of pre-discharge conference』was 3.65±0.87 out of 5. We conducted a multiple logistic regression analysis by using『Comprehensive achievement of pre-discharge conference』as the dependent variable. With respect to【Interprofessional cooperation factors】,『Ease of day-to-day cooperation with acute hospitals』and『Frequency of information sharing』are involved in『Comprehensive achievement of predischarge conference』. In addition,『Timing of conference』,『Effective utilization of conference material』and『Trust relationship between team members』,『Realization of support plans by team』and『Participation of patients』are involved in the achievement of the pre-discharge conference as to【Components of pre-discharge conference】. |
