手術用器械台はアルコール綿で清拭後, 器械台カバーを用いているが, メーヨー板は滅菌している. 滅菌したメーヨー板を術式に応じて準備することは, 保管場所の確保, 滅菌期限の確認, 補充などコスト高であり, 非効率的である. 感染予防策の改善, 簡略化とコスト削減に向けて, メーヨー板を手術用器械台と同様の方法に変更できないかと考えた. [検討１] 術中にメーヨー板に掛けたオイフに穴があくか否かを検討した. 結果49件中13件のオイフに穴があいていた. メスや縫合針などがオイフに当たり, 穴があいたことが考えられたため, 防止策として[検討２] メーヨー板に掛けたオイフの上にラバーシートを用いて検討した. 結果44件中３件で穴があいていた. いずれもメスや縫合針などが手術中にラバーシートを貫通してオイフに達したものと考えられた. 従って, メーヨー板の滅菌を省略するのは, 脳外科や心臓外科などのクラスⅠ/clean 手術においては推奨できないものと考える. |

At our hospital, the instrument table is wiped with an antibacterial wet cloth prior to the start of surgery, and an instrument cover is used. The Mayo table is sterilized before use. In an effort to simplify the sterilization process and reduce costs, we wanted to clarify whether it would be possible to omit the sterilization of the Mayo table, and use a method of wiping it instead, in the same way as the instrument table. First, in [Study 1], we looked at whether or not holes were created in the cover cloth placed on the Mayo table during surgery. In [Study 2], a rubber sheet was placed over the sheet placed on the Mayo table. The results showed colored water marks in 13 (26.5％) of the 49 cases in [Study 1]. Based on the position and number of marks it is believed that scalpels, suture needles and other sharp objects come into contact with the cover cloth during surgery, creating holes. In [Study 2], three (6.8％) cases were found among the 44 cases in which colored water marks were created. It is believed that scalpels, suture needles and other sharp objects penetrated the rubber sheet during surgery, creating holes. Based on the present findings, it is believed that placing a rubber sheet on the unit may not be applicable for class I/clean operations, and the Mayo table must indeed be sterilized prior to each surgery. |
