| 内容記述 | リハビリテーションにおいては, 障害概念の検討は最大の関心ごとの一つである. WHO は2001年に, 国際生活機能分類(ICF) を採択し, 新しい障害概念を提起した. 本稿では, このICF モデルに主観的次元を加えた上田の修正ICF モデルを, より普遍的でわかりやすい形に発展させた障害概念と修正ICF モデルを提示する. その特徴は, 第１に, 上田の修正ICF モデルの主観的次元の構成要素である｢主観的体験｣に替えて, 緩和ケアを参考に｢スピリチュアリティ｣の用語と定義を採用したことである. 第２は, ｢スピリチュアリティ｣の否定的側面を｢スピリチュアリティ低下｣として, ｢スピリチュアリティ｣に対応させたことである. 第３は, ｢スピリチュアリティ｣をICFモデルのすべての構成要素と双方向で直接結びつけたことである. この修正ICF モデルを用いることで, 患者や障害者の精神的自立(自律) をめざすリハビリテーションにより貢献できると考えられる. | |||||||||

| 内容記述 | Research on the concept of “disability”is one of the most interesting themes in the field of rehabilitation. The World Health Organization adopted the International Classification of Functioning, Disability, and Health (ICF) which provides new definitions of “disability” and its conceptual model in 2001. The ICF model has many advantages compared to the previous model. However, Dr. Ueda has maintained that the subjective dimension of the model is still necessary, and has proposed a modified ICF model including “subjective experience.” The purpose of this study was to examine the modified ICF model, to generalize the model by adopting the most popular concepts of the subjective dimension, and to develop a new modified ICF model for rehabilitation. The new modified ICF model has three distinct features compared to Dr. Ueda’s model : 1) The conversion from Ueda’s “subjective experience” to “spirituality” as a new term and definition of the subjective dimension in the model. The idea of “spirituality” comes from palliative care. 2) The adoption of “spirituality-associated limitations” as a negative side of “spirituality.” 3) The connection between “spirituality” and all six factors in the original ICF model with two-way arrows between them. The new modified ICF model will make medical and care workers more aware of the patients’ spiritual concern, and will facilitate the patients’ mental independence for rehabilitation. | |||||||||
