認知症者に機能訓練やADL訓練を行うときに問題となる動作や行為の特徴をとらえるため、介護老人保健施設入所者の動作・行為の特徴と知的能力及びADLとの関連について検討した。対象は秋田県内の介護老人保健施設Aに入所し、訓練やレクリエーションに参加している10名である。実施した検査はHDS-R、CDR、BI(横浜市大改訂版)、SPTAである。SPTAの検査場面はVTRにて撮影し、動作の誤反応を分析した。知的能力が重度であっても日常生活動作の自立度が高い対象者がいた。SPTAの誤反応の出現が多かった項目の特徴は、普段あまり行わない動作や対象物を扱わない動作であった。誤反応の少なかった項目の特徴は、過去によく行っていた動作もしくは現在も行っている動作等で、動作に意味のあるものが多かった。指示様式別の特徴では、口頭命令で誤反応が現れても、模倣や道具使用で正常反応が得られる対象者が多かった。以上より、認知症へのSPTAの実施が有用であることが示唆された。
To characterize the activities and behavior of individuals with dementia possibly causing problems during functional rehabilitation or training on activities of daily living (ADL), we analyzed the relationship between the characteristics of the activities and behaviors and the intellectual ability and ADL of elderly individuals accommodated in a geriatric health services facility. The subjects were 10 individuals staying at the geriatric health services facility A located in Akita Prefecture, and participating in rehabilitation/training and recreational programs. The following tests, namely, the HDS-R,CDR,BI(version modified by Yokohama City University) and SPTA, were undertaken in each of the subjects. The scenes during the SPTA were recorded on VTR for subsequent analysis of erroneous responses in the activities made by the subjects. In some subjects, the intellectual ability was severely impaired but highly independent activities of daily living were possible. In the SPTA, erroneous responses were often seen for activities that the subjects were unfamiliar with during daily living and activities involving no objects or materials. Erroneous responses were less frequent in the activities that the subjects had often engaged in previously or were still practicing (many of these activities were meaningful activities). When the characteristics were analyzed in relation to the style of instructions, there were many subjects in whom normal responses were obtained with mimicking or use of tools even though erroneous responses were obtained to oral instructions. These results suggest that SPTA is useful in the management of dementia. |
