| 内容記述 | 腎移植後のメタボリックシンドローム(MetS) は腎グラフト機能を悪化させると考えられるがその発症の実態は 明らかではない. 腎移植後のMetS 発症の実態を明らかにすることを目的とした. 2004～2011年に, 秋田大学医学部 附属病院で腎移植を受け, 移植後２年以上経過した患者103例を対象とした. 調査項目は患者基本情報, 血液生化学 的検査, 血圧, 体重, BMI, 服薬状況, eGFR. 分析方法には, Mann-Whitney U 検定, χ2 検定, 多重ロジスティッ ク回帰分析を用いた. その結果, 移植後にMetS と診断された者は30.1％であった. これは, 一般の日本人の発症率 より高値であるが, 欧米の腎移植患者における発症率より低値であった. 移植後にMetS となる者の特徴を移植前の 要因に求めると, BMI 高値, 脂質異常症であった. また移植後の要因では, 体重増加率と高尿酸血症であった. 従っ て移植施設の看護師は, MetS の状況や尿酸値等を定期的に調べながら, 医師や栄養士と連携して, その時々に合っ た効果的な移植後患者指導を行っていく必要がある. |

| 内容記述 | It is believed that metabolic syndrome (MetS) following renal transplantation exacerbates the function of renal grafts ; however, the details about the onset of MetS after transplantation have not been clarified. The objective of this study was to clarify the characteristics of the onset of MetS after renal transplantation. Subjects : The subjects included 103 patients who underwent renal transplantation at Akita University Hospital from 2004 to 2011 in which two years or more had passed after transplantation. Investigation items : (1) Basic patient information, (2) blood specimen (a biochemical examination), (3) sBP/dBP, body weight, BMI (kg/m2), (4) drug intake, and (5) eGFR. Analysis methods : The Mann-Whitney U test, an χ2-test and a multiple logistic regression analysis were performed. In total, 30.1％ of the patients were diagnosed with MetS following transplantation. This rate is higher than the onset in the general Japanese population, but lower than that in renal transplantation patients in Western countries. The following pretransplantation factors were associated with the onset of MetS after transplantation : high BMI ; dyslipidemia and after transplantation, body weight increase, and hyperuricemia. Nurses in transplantation institutes must periodically identify MetS and uric acid levels and carry out effective lifestyle guidance in cooperation with doctors and nutritionists. |
