精神科病棟に入院した初回の自殺未遂患者の家族への効果的な支援について検討するために，自殺未遂患者の家族の思いを明らかにすることを目的として研究を行った．４名の家族を対象に退院時に半構成的インタビューを行った．その結果，【衝撃ととまどい】【負担感】【自責感】【相手への苛立ちとはがゆさ】【安心感の確保】【回復への期待感】【チャンスに変える】【思いやり】【再企図の不安】【決意と覚悟】【事が事だけに誰にも話せない】【語りによる安堵】という12のカテゴリーが抽出された．初回の自殺未遂患者の家族の思いは，インタビューの最初は自殺企図への反応と誰にも話せないという思いから，再企図への不安とまた一緒に暮らす覚悟へ，そして語れたことによる安堵と変容していた．看護師による患者家族へのインタビューはナラティブアプローチになり，家族が患者と向き合うことを支援する可能性が示唆された．
This study aimed to evaluate an effective support for family members of patients who were hospitalized in psychiatric wards after attempting suicide for the first time and to clarify the thoughts of the family members. Semistructured interviews were conducted with 4 family members at the time of discharge of the patient. Based on the results, the following 12 categories were extracted: shock and confusion, sense of burden, self-blame, irritation and impatience, acquiring a sense of security, expectation of recovery, converting to another chance, consideration, anxiety regarding further attempts, determination and resolution, inability to express due to circumstances, and relief from being able to discuss. The thoughts of the family members of these patients changed based on reactions to suicide attempt, thoughts on not being able to express, anxiety regarding further attempts, resolution to live together again, and relief at being able to discuss. Nurses conducted interviews with family members of patients using a narrative approach, indicating the possibility of assistance in dealing with such patients. |
