| 内容記述 | 母乳育児に関わる助産師の経験知の一つとして,乳腺炎などの乳房トラブルに先行して乳児が授乳拒否を示す現象が言われている.そこで,そのような現象が実際にみられるのかどうか,実態調査を行った.生後2か月～3歳までの乳幼児の母親105人を対象に授乳時の乳児の行動や乳房トラブルの経験について自記式質問紙調査を実施した.その結果,授乳拒否を経験した母親は63人(60%)おり,そのうち24人(38%)は授乳拒否後に乳房痛,しこり,発赤等の乳房トラブルを経験していた.乳児の栄養法別にみると,授乳拒否の経験は母乳栄養の母親が混合栄養の母親より有意に多かった(p<0 .01).母乳栄養の母親では.授乳拒否の経験のある母親が経験のない母親に比べて,乳房トラブルを経験している割合が有意に高かった(p<0 .01). 乳児の授乳拒否と乳房トラブルには,関連のあることが示唆された.このことは,授乳拒否が乳房トラブル予知の重要な手がかりとなり得る可能性を示している. Some midwives, based on their experience of child care, say that babies sometimes refuse to suck prior to occurrence of mastitis, but there is little supporting scientific evidence. This study was aimed to investigate whether a baby's refusal to suckle is actually associated with mother.s mastitis-associated symptoms afterwards. We obtained questionnaires from 105 mothers of infants aged from 2 months to 3 years about their experience of their baby's refusal to suckle and mastitis-associated symptoms such as breast pain, swelling and redness. As results,63(60%) of mothers experienced their baby's refusal to suckle and 24(38%) of them underwent mastitis-associated symptoms afterwards. Refusal to suckle was more common among mothers who breast-fed (p<0.01). Among mothers who breast-fed, experience of mastitis-associated symptoms was more common in mothers with experience of refusal to suckle (p<0.01). In conclusions, there was a statistically significant relationship between a baby's refusal to suckle and subsequent mastitis-associated symptoms in the mother, and refusal may therefore predictthe close future occurrence of mastitis in the mother. |
