内容記述 Acid sphingomyelinase (ASM) is activated in various human diseases. Secretion of ASM (S-ASM) is accompanied by its activation, which clinically leads to elevated S-ASM activity in blood and urine. Here, serum and urinary levels of S-ASM activity were determined in patients with chronic diseases, which were divided into non-renal and renal groups depending on the etiology of the diseases. In 28 patients with non-renal diseases, serum and urinary S-ASM activities were 7.40 ± 0.28 nmol/ml/h and 0.28 ± 0.35 nmol/mgCr/h, respectively. A significant correlation was found between the two S-ASM activities in this group (r = 0.539, p < 0.05). Conversely, among 12 patients with renal diseases, serum S-ASM activities were not elevated (3.99 ± 1.48 nmol/ml/h); however, urinary activities had increased to 0.35 ± 0.50 nmol/mgCr/h. Fractional S-ASM excretion (FE S-ASM) was calculated for both groups, yielding 0.011 ± 0.01 % in non-renal group and 0.030 ± 0.045 % in renal group. Individually, a patient with IgA nephritis demonstrated significantly elevated urinary S-ASM activities and increased FE S-ASM activity, suggesting aberrant dynamics of S-ASM in the kidney caused by the pathogenesis of IgA nephritis. This study indicates that urinary S-ASM activity could reflect the pathological state of renal diseases.
