Although various biomarkers have been proposed to monitor health in managing obesity, further exploration of key biological factors is needed in clinical practice.\nObjectives: This study aims to evaluate serum secretory acid sphingomyelinase (S-ASM) activity as a biomarker for obesity and its related conditions.\nMethods: Forty-seven individuals with obesity (percentage of overweight (POW) ≥ 20%) were divided into groups with mild to moderate obesity (POW < 50%) and severe obesity (POW ≥ 50%). Serum S-ASM activity was measured and compared between these groups. Correlations with liver enzyme ratios and S-ASM's diagnostic performance for obesity-related liver dysfunction were assessed using receiver operating characteristic curve analysis.\nResults: Serum S-ASM activity was higher in the severe obesity group( 7.2 ± 3.6 nmol/ml/h) compared to the mild to moderate obesity group( 4.7 ± 2.2 nmol/ml/h, p=0.03). Serum S-ASM activity negatively correlated with the aspartate aminotransferase/alanine aminotransferase ratio( r = -0.389, p < 0.01).
