| 内容記述 | A 59-year-old man presented to the emergency department with chest pain and persistent fever for 2 days. Transthoracic echocardiography revealed severe aortic regurgitation and a vegetation-like structure on the aortic valve which protruding into the left ventricular outflow tract. Although contrast-enhanced computed tomography revealed enlargement of the aortic root of 47 mm in diameter, dissection findings were not noted. We diagnosed infective endocarditis of aortic valve and performed an urgent surgical treatment. However, intraoperative transesophageal echocardiography revealed a circumferential dissection confined to the aortic root and the protrusion of the intimal flap into the left ventricular outflow tract, with no evidence of aortic valve vegetation or degeneration. Consequently, we made the true diagnosis of acute type A aortic dissection localized to the aortic root. An aortic root replacement using On-X 21 mm was successfully performed, and the postoperative recovery was uneventful. Localized aortic root dissection is difficult to diagnose because it is rare and can easily be missed on transthoracic echocardiography or contrast-enhanced computed tomography. In cases where transthoracic echocardiography and contrast-enhanced computed tomography fail to provide conclusive evidence of aortic dissection, transesophageal echocardiography should be considered as a further diagnostic approach. |
