 note = {インスリン治療は, 指示の多様性や注射指示から実施までのプロセスの複雑さ等が相俟って, インシデントにつな
がる事が少なくない. われわれは, 医師, 看護師合同によるインスリン治療の安全対策チームを組織し, インスリン
治療の指示の出し方に関する標準化を実施してきた. しかし, この標準化後もインスリン注射インシデントは発生し,
その要因のひとつに, インスリン指示書に係る問題があると考えた. そこで, 医師の指示記載部分の簡略化ならびに
患者との情報の共有化を重要視してインスリン指示書を改良し, その効果を検討した. 新しい指示書を導入する以前
の６ヵ月間と導入後の６ヵ月間のインスリン注射インシデントの発生数ならびにその内容を比較した. インシデント
の発生数は, 導入前46件, 導入後37件であり, 有意差はなかった. 一方, インシデント内容を見ると, 投与速度間違
いや投与日時間違いなどの情報処理上のインシデントが, 新しい指示書導入により, 28件から15件に有意に減少した.
しかし, ｢注射し忘れ｣は, 導入前18件, 導入後22件と差は認めず, 今後の課題である.
Insulin-related incidents are not uncommon due to the variety of prescriptions, the number of insulin
products and the complexity of the process from prescription to injection. We organized an insulin therapy
safety management team consisting of physicians and nurses, which standardized insulin therapy.
However, insulin related incidents did not decrease thereafter, and the defect of the insulin ordering form
is considered to be one of the causes.
The insulin ordering form was revised with emphasis on simplifying the section for the doctor s directions
and joint ownership of information for patients, nurses and doctors, and introduced clinically on
August 1, 2007.
The total number and content of insulin related incidents collected during the 6 months before introduction
(2007. 2. 1～2007. 7. 31) and after introduction (2007. 8. 1～2008. 1. 31) of the revised insulin ordering
form were evaluated comparatively.
The total number of insulin related incidents was not changed by the introduction of the revised
ordering form. However, incidents relating to drug administration such as injection speed and time of
injection decreased significantly from 28 to 15. On the contrary, no change was recorded for the number
of incidents of  forgetting injection , and this should be tackled in future.},
